    Post-pregnancy depression is an important medical intervention for new moms as it is a massive life transition on every level. Some women have great support and manage fine while others struggle with connecting to the baby or have depression and low moods. Women are not offered this service and they rarely even get time to focus on the fact that this transition might be challenging.  

    Mansi Poddar, a Trauma informed Psychotherapist says, “Many women struggle with body image issues, issues with their partner and his family, gender dissatisfaction etc and these need to be dealt with so that mom and baby can attach securely and the woman feels supported and not lonely in her PPT journey.”

    “Most women do not even realise they might be struggling with PPD or other challenges. If they do not have a supportive home environment or partner the challenges get even more and mental health intervention is crucial. There is a deep shame surrounding having depression or not being able to attach to the baby, adds Ms Mansi.

    Most women feel stress and this impacts their milk supply which in turn worsens the shame and low mood. Doctors and hospitals need to initiate a conversation about PPD and postpartum challenges. For eg Mansi highlights, “Women who have pre-term babies or pre-eclampsia might need immediate support to cope. Any birth difficulties can cause severe emotional and psychological challenges and are rarely spoken about.”

    Tips for Supporting Your Partner Post Pregnancy

    Here are some tips on how to cope with PPD:

    – Seek the support of someone who has experience with women’s health. Someone who also has lived experience can be useful. 

    – It’s important to have the therapist do couples and family sessions since the depression also stems from the system and if there is a lack of support it can get harder 

    – Talk to your doctor if the symptoms are severe or aren’t settling after a few weeks. 

    – Men need to learn how to hold space for women and emotions. Avoid being “positive” or asking her to look on the bright side. Many shut down due to helplessness. This is going to worsen the depression. Ask her how she feels and allow the space to emote and share. Even if it’s around your parents. Avoid getting too defensive 

    – Get breaks from the baby. Yes. You must get some time to sleep. Eat or even go out or do something you love.

    Neurotransmitters Produced By Gut Bacteria Influence The Development Of The Newborn Immune System: Study


    The preclinical study, published in Science Immunology on March 15, showed that bacteria abundant in the guts of newborns produce serotonin, which promotes the development of immune cells called T-regulatory cells or Tregs. These cells suppress inappropriate immune responses to help prevent autoimmune diseases and dangerous allergic reactions to harmless food items or beneficial gut microbes.

    “The gut is now known as the second human brain as it makes over 90 percent of the neurotransmitters in the human body. While neurotransmitters such as serotonin are best known for their roles in brain health, receptors for neurotransmitters are located throughout the human body,” explained the study’s senior author, Dr. Melody Zeng, an assistant professor of immunology in the Gale and Ira Drukier Institute for Children’s Research and the Department of Pediatrics at Weill Cornell Medicine.

    Gut Bacteria in Babies Provide a Helping Hand

    The researchers observed that the neonatal mouse gut had much higher levels of neurotransmitters, including serotonin, than the adult gut. “So far, almost all studies of gut neurotransmitters were conducted in adult animals or human subjects, where a specific gut cell type called enterochromaffin cells produce neurotransmitters,” said Dr. Zeng. “However, we discovered that this isn’t the case in the newborn gut where most of the serotonin is made by bacteria that are more abundant in the neonatal gut.”

    This was also confirmed in babies through a human infant stool biobank that the Zeng lab has established in collaboration with the Neonatal Intensive Care Unit in the NewYork-Presbyterian Alexandra Cohen Hospital for Women and Newborns. These samples were obtained with parental consent and de-identified.

    The study results suggest that before the neonatal gut is mature enough to make its neurotransmitters, unique gut bacteria may supply neurotransmitters that are needed for critical biological functions during early development.

    “We found that gut bacteria in young mice not only directly produce serotonin but also decrease an enzyme called monoamine oxidase that normally breaks down serotonin, thus keeping gut serotonin levels high,” said the study’s lead author Dr Katherine Sanidad, a postdoctoral associate in pediatrics at Weill Cornell Medicine.

    The high serotonin levels shift the balance of immune cells by increasing the number of Tregs, which helps prevent the immune system from overreacting and attacking gut bacteria or food antigens. “The neonatal gut needs these serotonin-producing bacteria to keep the immune system in check,” Dr. Sanidad added.

    Healthy Immune System Helps Later in Life

    Dr Zeng noted that this work underscores the importance of having the right types of beneficial bacteria soon after birth. Babies in developed countries have better access to antibiotics, less exposure to diverse microbes in their clean environments, and potentially unhealthy diets that may significantly impact the abundance of serotonin-producing bacteria in their intestines.

    As a result, these babies may have fewer Tregs and develop immune reactions to their own gut bacteria, or allergies to food. This may be one reason food allergies have become increasingly common in children, particularly in developed countries. “If educated properly, the immune system in babies would recognize that things like peanuts and eggs are okay, and it doesn’t have to attack them,” she said. This may also have an impact on developing autoimmune diseases–when the immune system attacks the body’s own healthy cells–later in life.

    The team next plans to look at bacteria in human infant stool samples to measure their production of serotonin, other neurotransmitters, and molecules that may help train the immune system to prevent future immune-related diseases, such as allergies, infections, and cancer.

    “It’s essential to understand how the immune system is trained during early life, but this is understudied in newborns and children. Further studies of these developmental periods may hopefully lead us to mitigation approaches to reduce the risk of inflammatory diseases like food allergies and inflammatory bowel disease later in life,” Dr Sanidad said.

However, its applications have expanded to include elective fertility preservation and delaying menopause. This article explores the nuances of OTC, comparing it to traditional egg freezing and delving into its benefits, procedures, success rates, and associated costs.”

    Ovarian Cortex Freezing vs. Egg Freezing: Are they similar? 

    Unlike traditional egg freezing, “OTC does not require hormonal stimulation. The absence of ovarian stimulation enables the procedure to be performed promptly, without the need for multiple cycles. OTC allows for the freezing of hundreds to thousands of immature eggs, providing a more robust fertility preservation solution”, highlights Dr Firuza.

    Ovarian Cortex Freezing (OTC): Procedure and Retransplantation

    Dr Firuza explains, “The OTC procedure involves a laparoscopy to remove a portion of the ovarian cortex. The extracted tissue is meticulously processed in the laboratory, where it is frozen using a slow freezing technique. The frozen tissue can later be retransplanted orthotopically or heterotopically, near the fallopian tube or on the abdominal wall. The regrowth of ovarian tissue post-retransplantation leads to the production of natural hormones, preventing early onset menopause.”

    What is the downtime with OTC?

    “One of the notable advantages of OTC is its minimal downtime. After the laparoscopy, women can resume light activities the following day and return to full activities within 2 to 3 days, making it a convenient option for many”, adds Dr Firuza.

    What is the world-wide success rate with OTC?

    Dr Firuza explains, “Despite its low usage rate (8.7%), OTC boasts a high success rate after autotransplantation, with an 86% restoration of ovarian function and a 57% live-birth rate, showcasing its efficacy as a fertility preservation method.”

    Is there any age limit for OTC procedure?

    For elective or social freezing, OTC is typically offered until the age of 36. “Beyond this age, the quality and quantity of remaining eggs decline significantly, with the woman’s Anti-Müllerian Hormone (AMH) becoming a crucial factor in determining the appropriateness of OTC”, says Dr Firuza.

    “Apart from increasing pregnancy chances, OTC contributes to hormone production. The natural hormones produced by this method may offer advantages over medical Hormone Replacement Therapy (HRT), providing an additional dimension to its benefits.”

    Dr Firuza concluded, “The cost of OTC includes the laparoscopy procedure, ranging from Rs. 1 lakh to Rs. 3 lakhs, depending on hospital services and additional procedures. The freezing of ovarian tissue costs approximately Rs. 2 lakhs, covering the first year of ovarian tissue cryostorage. In comparison to two cycles of ovarian stimulation and egg freezing, OTC proves to be a cost-effective option.”
